Reenrollment Process

We sent reenrollment instructions to current families on April 5, 2021; these instructions are also posted below. To retain their space at Creative Minds for the 2021–22 school year, families must reenroll through our school by May 3.

How to Reenroll

There are three options for reenrolling: online, at school, or through a video (Zoom) appointment. To reenroll at school or on Zoom, please click here to schedule an appointment.

Reenrollment consists of three steps: completing the enrollment forms, submitting proof of residency, and signing the DC Residency Verification Form.

Online reenrollment should take about 10 minutes per child. To reenroll online follow these steps:

  1. Visit https://cmipcs.powerschool.com/public.
  2. To log in to PowerSchool, follow the indvidualized instructions that were sent the week of March 29.
  3. From the left column, select FORMS.
  4. Complete the forms. Note that many forms will be prepopulated, but you can update information, if needed. Click the blue SUBMIT button at the bottom of the form.
  5. On the upload page, submit these documents:
    1. Proof of DC residency
    2. Parking-pass information (if applicable)
    3. Universal Health Certificate (if the certificate on file at school is more than a year old)
  6. Sign the DC Residency Form
    1. Option 1: Sign securely via DocuHub. This link will be sent via email.
    2. Option 2: Sign forms at the school.

If you prefer not to upload documents, you may email them to us, at frontdesk@cmipcs.org, or drop them off at school.

 

Hay tres opciones para reinscribirse: en línea, en la escuela o a través de una cita de video (Zoom). Para volver a inscribirse en la escuela o en Zoom haga clic aquí para programar una cita.

La reinscripción consta de tres pasos: completar los formatos de inscripción, presentar un comprobante de residencia y firmar el Formato de Verificación de Residencia de DC.

La reinscripción en línea debe tardar unos 10 minutos por niño. Para volver a inscribirse en línea siga estos pasos:

Pursuant to The D.C. School Reform Act of 1995 (Sec 38-1802.06) as amended, enrollment in public charter schools is open to all students who are residents of the District of Columbia, and if space is available, to non-resident students who pay tuition at the rate established by the Office of the State Superintendent of Education.

Penalty for False Information

Any person, including any District of Columbia public school or public charter school official, who knowingly supplies false information to a public official shall be subject to payment of fine of not more than $500, or imprisonment for not more than 90 days, or any combination thereof. The case of any such person may be referred to the Office of the Attorney General for consideration for prosecution.

The Family Educational Rights and Privacy Act (FERPA)

The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that Creative Minds International Public Charter School, with certain exceptions, obtain your written consent prior to the disclosure of personally identifiable information from your child’s education records. However, Creative Minds International Public Charter School may disclose appropriately designated “directory information” without written consent, unless you have advised the LEA to the contrary in accordance with LEA’s procedures. The primary purpose of directory information is to allow the Creative Minds International Public Charter School to include this type of information from your child’s education records in certain school publications.

If you do not want Creative Minds International Public Charter School to disclose directory information from your child’s education without your prior written consent, you must notify the LEA in writing by filling out the form provided by the School in the Welcome Packet and returning it on the first day of school in the fall of 2020.
